Family Registration Procedures
If there is a birth or death in your household while you are in Japan,
you must notify the city office. You can also file a notification if you
marry or divorce.
Childbirth Report
If a child is born in your family, please submit a Childbirth Report
(with a Birth Certificate completed by the doctor or midwife) at the city
office of the place of birth or of the registered address where the reporting
person resides within 14 days after the birth.
* Main Procedures to be Completed with the Birth Notification:
- Foreign Resident Registration (at the Civic Affairs Division of the
city office within 60 days of birth)
- Application for a Passport (at the embassy of your home country)
- Application for a visa (at the Immigration Bureau within 30 days
after the birth)
- Visa Acquisition Notification (at the Civic Affairs Division of the
city office within 14 days of acquisition)
- 5Procedures required by your home country (In some cases, there are
stipulated procedures that are required by the embassy or consulate
office of your home country. Please call the embassy or consulate office
of your home country.)
Marriage Report
If a spouse is Japanese, register the marriage at the municipal office
where the Japanese person's family register is kept or the current address.
If the marriage is between two foreign residents, please file a Marriage
Report at the municipal office of the registered address of either spouse.
- Marriage Report (available at the city office)
- Certificate of Entitlement to Marriage (issued by the authorities
of one's home country, embassy in Japan, etc.)
- Birth Certificate with translation
- Passport and proof of foreign resident registration
* Please include the name of the translator on the translation.
* If the embassy of your home country cannot issue a Certificate of
Entitlement to Marriage, please call the Civic Affairs Division.
Death Report
(1)Reporting Period
Within seven days of the death
(2)Office
Municipal office of the city, ward, town or village of the deceased or
of the reporting person.
(3)Necessary items
- Death Report with Death Certificate (one copy)
- A written application for a cremation permit
* The foreign resident registration card of the deceased must be
returned.
Issuance of Receipt of Report Certificates
Certificates proving that notification reports, such as Birth, Death
and Marriage, etc. have been duly accepted according to the law, can be
issued at the city office where the notification report was submitted.
